Please designate a Program Initiative, Foundation, or disease in the memo section of the check or in a cover letter.

If your donation is in honor or memory of someone, please include that individual's name with your donation. If you include the honoree's family's address, we will send an acknowledgement letter to them indicating that such a donation has been made.
